If you need to submit a claim, please select the appropriate Loss Report below. Simply fill out the report, include all documents needed for submission, and either mail it or fax it to our Claims Department at 770.396.0147 Please refer to our Forms Instructions for all necessary documentation. Vehicle Report of Loss Claim Applications Please print an application or simply fill out online. If submitted online, it will go directly to our Sales and Service Department for review. You will receive a confirmation of receipt within 2 business days.
